It is assumed that you don't have to pay for food since you adventure for 2 of the 4 days. Say I want to only eat Elven Trail Rations, which cost 2 gold (4x a normal meal). Is this free, or would I have to pay 1.5 gold daily?

I don't know about official stance but I generally treat adventurers as having the Poor Lifestyle unless they actually buy a higher level. Rations are usually only recorded while they're out of town, but I assume they're able to scrounge up some bread, cheese, and fruit when they're in town or that at least those minimums are included with their lodging price. If the character wants to have a more elaborate lifestyle I make them pay for it.

You must pay for special rations. Cost of Living is usually ignored because it provides no mechanical benefit and is a pain in the ass to track. If you're deriving mechanical benefit from your lifestyle, you must track the costs.
